First, I turned on Allow in the Background. Not sure where in the installation process I was supposed to allow this as I said yes to the Install Helper request every time, but whatever.
As I noted above, I think the fundamental problem was that it wasn't asking for permission to use the removable drive. I noted under Privacy -> Files & Folders that the option to allow this was grayed out because I had Full Disk Access turned on as previously suggested. By switching that off, rebooting to capture these changes, and then following LBJ2's startup instructions it did ask me for permission to access the removable drive. After allowing it, it started working after a few seconds delay.
As an aside, to add insult to injury the only reason I started down this path with this reader was because OWC customer service told me my new cards needed the firmware update. Now that Innergize is working I see that they don't....Show more →

"You should see something in System Settings --> General --> Login Items & Extensions --> Allow in Background that says "Other World Computing" and is enabled"
-> that fixed the issue. After enabling it, I got the prompt for the storage access.

I’m a little bit disappointed I went down this path with OWC despite the good deal on the cards. Especially since their high speed reader doesn’t have an option for a dedicated CFE-A card and as reported here the adapter heats it up quite a bit, as well as that reader not being a dual reader with SD capability.
I believe I saw where the Sony cards don’t require their reader to update their firmware? Mostly just curious since that means it’s at least possible if that’s the case. Their cards in that format are too expensive, but still curious.
Kind of wish I had just waited for a ProGrade or other brands sale. They’re bound to be coming down in price.
Oh well. Live and learn, perhaps it will turn out all good.

A cynical person might speculate OWC were well aware of issues and decided to offer at a big discount to get lot's of sales before word spread. Making a new reader that did not require adapter, updating cards before shipping and getting their story straight on whether cards shipped with adapters was deemed too costly.
So, anyone have complaints about the never heard of before Pergear?

I’ve had my Pergear 520GB for almost two years now and have had no issues. Take that as an n=1 study.
I do now have the OWC 480GB 4.0 and their 4.0 reader and A to B adapter and also have no issues.

Sorry if I missed it, but do we know if cards are shipping with up to date firmware now? Just got my first camera that supports CF-A, my storage needs are pretty modest but I wouldn't mind the increased download speeds.
If these are going to be a hassle it seems like Lexar Gold cards are pretty well reviewed on B&H without being too expensive. "Only" 800mb/s but I'm coming from UHS-I SD cards.

I purchased two of their cards at the end of March and I was concerned about the firmware. I contacted support and they told me any cards purchased this year should be good. I have been using the two cards without issue.
Thank you for sending the invoice. I can confirm that the memory card that you purchased does not require a firmware update, all of the memory cards that were purchased starting this year are expected to be working fine without any issues.
