I use the Peak Design Sling 10 now as on a good day I can get 5 lenses and body in it and it’s fits my back really nicely. Everything I use the Peak Design backpack 20. Than I got zillion I need to sell. Also for PR gigs I use the Tenba Copper 13 slim.

But suppose you're not working and you just want to take your camera with just a medium lens on it?

I have one of those Domke fx5 or whatever it’s called bags I had for like 15 years . It fits a body and 2 lenses if fairly small. I use this bag a lot for a long lens just to keep close by sometimes. Like my Batis 135

I would not mind having the Peak Design 5 bag instead though looks like it will do the same and even a extra small .lens plus it has pockets the Domke does not have a pocket.

Now being a bag whore causes many issues. Not only do you have a pile of bags in your closet which is bad enough but the flip side your spouse has two closets full of shoes to offset your spending. So your 80 dollar bag may cost you net 300-1500 per bag. Lol

I have been thinking about trying to carry my camera with me much more often than I now do. Part of the problem is the bag that one needs to have with one while also carrying and doing other things.

What is the ideal bag to carry a Sony A7xxx series camera with a medium lens mounted around with one all the time?

I am looking at the Domke 700 51B F-5XA that another FMer had shown photographs of in another thread:
https://smile.amazon.com/gp/product/B000PHOK04/ref=oh_aui_detailpage_o00_s00?ie=UTF8&psc=1

The Domke700 51D F-5XA is smaller, but I think a bit too small.

Any suggestions for a small, unobtrusive, protective, easy-to-access bag

I like the Think Tank Hubba Hubba Hiney for camera with lens. So many ways to carry it. Holds a small flash, small second lens, sandwich, but not much else.

Very Pricy, a little larger than you requested but I find it surprisingly functional and versatile. Billingham fStop 2.8. I like it with the optional shoulder pad and use it for anything from just the camera and lens to in addition two extra lenses, small flash, a few other odds and ends. When shooting, I can fold the top flap completely back ready to shoot but it still has a top flap that protects but easily moved out of the way. It is hard to explain. Back zipper, back inside pocket, front inside pocket. I fasten one outside flap quick and easy if I want to ensure I do not accidentally let the camera fall out.

A small issue but important to me is the strap. I like having a detachable and interchangeable strap. That is a nice feature of the Think Tank Mirrorless Mover bags (the Urban Disguise has an integrated strap). I remove the strap and pack the bag in carry on baggage. I use the Tom Bihn Absolute strap on my other personal item when in transit and then transfer the strap to the camera bag for day-to-day use. It cuts down on bulk which is a big deal if you are trying to be a carry on only traveler with camera gear.

My minimalist bag is the Lowepro Adventura 140, which is bigger than you want, but the 120 would be about right. Lowepro Adventura 120. It's a bit smaller than the ILC 100 but not as limiting as the ILC 50. If you don't like the looks of the orange racing stripes, the older version (eBay link) is a bit more nondescript but lacks the new version's composite bottom.

I have two Lowepro Passport Sling bags -- one in So Cal, one in Washington. Just so I'm never without.

FYI the Bihn Absolute is made by Op/Tech, who also make it in different sizes as the SOS.

As shoulder bags I have both the Billingham Hadley small and Hadley Pro. The Hadley small will take either the A7rIII with a large zoom lens or the A7rIII with FE55 + the Leica Q and a medium size iPad will fit as well plus wallet spare batteries etc. The Hadley Pro (which I have had for over 10 years and is well worn) will take 2 A7r bodies plus larger lenses and you can also add extra pockets on the end. The Hadley Pro also has a handle on the top flap which can make for easier carrying at times

If I'm hiking etc I'm using a Deuter Futura 26L backpack with a generic insert. Around town I'll use a Tenba Cooper 13 Slim....and really that's about it. The Tenba Cooper 13 Slim seems to work for most situations and has worn well the year or so I've had it.

One feature I insist on is a zipper, a good zipper. Some of these bags, like the Crumpler for example, don't have zippers.

I insist on a zipper because:

better dust protection. Some places (e.g. Africa, Southeast Asia) can be very dusty so zipping the bag shut helps with dust protection.

A zipper is a deterrent for thieves in crowded areas like trains, buses, markets, etc. Were I a pickpocket, I know where I would start when I size up the photo gear.

Packing. I try to travel as small and light as possible. Having a zipper on the bag allows me to comfortably overstuff the camera bag on transit days without fear of things falling out or being stolen. Well designed zippers can be locked as well.

The TT Mirrorless Mover bags have a nice quality zipper covered by a flap with a magnetic closure. It is a quality, thoughtful design for my purposes.

Leather Ona Bowery...I’ve carried 2 Leica bodies with 2 lenses in this bag all over the world and it’s beaten everything else I’ve tried.

I can vouch for both the Hubba Hubba Hiney and PD Sling for your purposes, OP. The PD doesn't need a cover to be fairly water resistant, which is nice, and the strap design is better thought out for quick adjustments, but it's less easy to detach completely if you're doing that a lot. I use the HHH mostly for backpacking on a belt, but I've used it a few times with the strap in town. I'm always surprised at how much it can fit without seeming cramped. The divider design on PD stuff is second to none, everyone else seems be in the stone age with those, but if you're only carrying one lens that probably of less concern to you.

Domke 5-FX: body, 35mm 2.8, 28mm f2. Could be a bit small for other lenses. Shoulder strap or wear on belt.

I use an A7rII with a 16-35/4 and a 55/1.8. The most minimalistic bag that will fit, that I like using, is a Think Tank Mirrorless Mover 20i. It is snug but not unpleasant to work out of. A Mirrorless Mover 30i will accommodate the addition of a 35/2.8, filter wallet, and some odds and ends.

In either case, the Tom Bihn Absolute Strap is a nice addition.

For your stated plans, the MM 20i would work well.

Think Tank Mirrorless Mover 20

For a minimalist one-lens bag, this one gets my vote. I initially got it for the a6300 with the 18-105 (which it fits like a glove) but I'm pleased that it also works with the a7R3 with the 24-70 f/4 and even with the 70-300 with the hood reversed.

A use a 22L Osprey pack as when I go out I take along more than just camera gear ( sun screen, bug repellent, sun glasses, jacket / sweater etc... )

To carry all this and my gear, a photo pack is just too uncomfortable.
