A huge soft box on the right but slightly forward of the model. You can see that in the catch light of the model, especially on the first one. And another light source at lower intensity with barn doors in the back at a higher position outside the frame and pointing down. The light in the back is just to create separation of the model from the background. OK, the bokeh balls are on the background material .
