You get used to the weight. I know I did.
Besides, people stare at me with any white lens, be it the 70-200, 300L f4 IS, or the 200L f2. People will always be people!
I actually can fit this lens with hood reversed in my LowePro Slingshot 200 along with my 1DMKIII, 2 smaller lenses like the 24L + 50L, and the 1.4x & 2s TC's.
It get tight in there and it sure is heavy, but it can be done!
It's a great lens, as so many of those photos above clearly show. But it's also a big bulky and heavy lens! The 200/2 makes the 70-200 2.8 IS look like a lightweight carryaround option.
I had the 200/2 on a monopod at Colonial Williamsburg around Thanksgiving last year and people were staring at me everywhere I went with it.
That lens is commitment in every respect. You've got to commit to paying for it and then to carrying it around. But the photos with it really are unique.
