I have been using a little OCF during the processional. Set up a light on manual to hit a specific place...pre-focus and shoot when they hit the spot. Other than the processional, not at all.
I have before. But never will again. Why risk inconsistent exposures for something that A) fast glass and today's iso can handle and B) is probably disrespectful in that environment (even in the entrance/exit).

It's not disrespectful if you ask permission first.
I don't know how dark the churches are you guys are shooting in, but a high-iso-capable body with fast primes works wonders... And actually I'm fine with my zooms at 2.8.
I have once as the wedding party was entering the church. This was before I had any fast telephoto prime and was using a 70-200 2.8. I shot with the flash at low power to fill in a little extra light. I ran it by the minister as well as the b&g beforehand. Haven't used flash at a ceremony since then.
No way during or before ceremony. Yes Only when Asked to do formals/large groups at the altar/in church.
