Google his name. If the Ed Mika you want is the one making adapters, there are several listings for him and you may be able to get in touch with him through ebay.

I know that this is an old thread, but going to give it a shot. I have a Canon FD 800mm f5.6L with an Ed Mika adapter. There was evidently an issue with earlier versions not being compatible with a Canon 5D. Does anyone know if that issue was ever resolved? Not exactly sure when I bought it. It was sometime in 2012.
