Lens includes both caps, tripod ring, soft case, and original box - i.e. pretty much everything. Only marks on the lens are a 2 very small rub marks on both the tripod ring and lens barrel where the ring/barrel meet. They can be seen in the picture. The glass is clear, but I would not rate it as "flawless" - it has small cleaning marks on both front and rear elements. As far as I can tell, it has no effect at all on image quality. They were already there when I purchased the lens last year for $700.
The picture of the rear element looks pretty scary... I tried to get as many of the marks visible as I could so there are absolutely no surprises when it comes in the mail. Again, I want to stress - image quality isn't affected. I tested this lens against a Canon 70-200 2.8 when I bought it and kept the Sigma - IQ was identical, and sigma was $150 less. Seriously.
