There is option on lots of cameras to adjust and align to make the images sharper.
Do you do that? is there a real need for it or it is just a gimmick to give you thje feeling that you did perfect your pictures?

Absolutely. Most of my lenses didn't need any adjustment. The couple that did had a noticeable improvement in IQ

I have nikon lenses and sigma for nikon. I am afraid I would mess it all up.
Do you do the adjustment in the camera or in the lens?

In the camera - assuming your camera has the feature to do this, as not all cameras have it.

And can you undo it to go back to the original?

Yes - it is very easy to go back.

Absolutely. It's not 'cool' to care this much about your equipment and delve into testing it but for fast lenses sometimes it's the only way to get the best results because of camera manufacturers' very poor manufacturing tolerances.
