A few years ago, I had Son Pham shave the mirror on my 5D for this lens. I've since moved on to the 5DMKII and I don't want to bother shaving the mirror on the latter. I've used the lens with a Novoflex adaptor (non-focus confirmation).
I'd say it's in Very Good condition. There are a few small abrasions on the edges of the focusing ring fins. Other than that, to my eyes it looks pristine: front and rear elements are free of scratches or loss of lens coating, blades are clean and f stop clicks are solid, front and rear caps included.
Since I purchased the lens, one of the release buttons on the lens shade has loosened a bit, so you have to remove the shade with care. Otherwise, the button can shift out of position on the attachment ring. The folks at E.P. Levine in Boston suggested that I apply a bit of superglue to anchor the attachment ring to the hood, rather than pay Leica to fix it. It's up to you.
I bought a Leica lens case from KEH that was advertised as the case for the 19mm. I don't think it's the original style case, but everything fits it it.
