I would like to see just a bit more fill. There is a slight look of a 'black eye' on the shadow side. If it wasn't for her beaming smile, the image would seem almost dramatic which isn't something that I would expect to see in a doctors office.
I actually like the crop. It keeps her eyes in the top of the image and it makes her look like she is sitting up straight and is full of energy.

When I do a shoot, I get the "needed" shots, then have a few minutes of fun.
I had done main/fill/bg/reflector and liked them, but I also liked the single light shots.
