I use a small aluminum strip with 3 holes in it, to mount 2 flashes on an umbrella swivel. Total cost was 0. The strip came out of the waste bin and the brass hardware came with the Manfrotto lightstands and swivels I already had.

I like the FourSquare from http://www.lightwaredirect.com. Far more versatile than any of these options suggested so far and you can now use your existing softboxes from Westcott, Chimera, Creative Lighting, Photoflex, etc. with it.

Ooh... that's very nice. But $249 is a bit higher than I would've ever guessed.
I saw several DIY rings like that on flickr rigged for 3 speedlites, but they had the heads close together, peeking through the ring (which I think would be easier to adjust flash settings, wake from sleep, change batteries, etc)
Here's a bracket I fabricated...basically the same thing Al Jacobs builds, but I welded a "shelf" onto it and cut a hole in the back so I could mount a CSRB+ receiver on it.

I'm using his product here. I have an umbrella bracket attached to a super clamp and strobing a bike race. Not a great picture. I would recommend it and you're right the price is good.
