A shutter is like $300? It's not that bad of a cost at all compared to how many shots they last.
But you have to remember other things have been actuated (mirror, mirror springs, actuator motors, shutter release, etc) many times so when do they break? Just putting a new engine in a car with 200,000 miles does not ensure it will go another 200,000 miles.

Weird, doesn't this "guaranteed to do 200.000" clicks mean anything then?
Is this only during that warranty period?
Nope, it's just a rated guide that it can.
A friend of mine bought a new Canon 1D Mark 3, got it delivered on a Monday and the shutter was broke 10 days later after a 1000 or so clicks. It was replaced under warranty though.
