Does anyone have samples of release forms for clients to make prints from a supplied CD?
I've Googled to try and find one, but haven't found anything suitable.
Fortunately, most labs won't print professional photos without a photographer's release.
I generally don't even offer files on a CD, but if I do, I need to have forms available for the client so that they can get prints made.
Any help would be appreciated.

I just include it in the license. I include a text file on the CD saying they have permission to print, and I print a line on the CD label itself saying "images licensed to <client> for printing"

I print my release right on the CD.
"Bearer of this CD is granted unlimited, personal, non-commercial print rights... blah blah blah. All Rights Reserved, Copyright...."
or something like that.
