I just have never liked it. Don't like the focal length. Mine isn't very sharp at all wide open. It's a $1100 lens that I never use. My wife (2nd shooter) uses it, but only about 10% of the time.
Color Munki. Nearly $500 just sitting on my shelf.
Represented to do it all----it doesn't work for me. Color Munki (at the moment) does not support icc version 2---only icc version 4. So---monitor profiles for me on a PC and using Capture One Pro don't work. No returns by B&H Photo. No solution from X-rite other than to tell me to buy yet another product from them. (Oh, yeah---and telling me to stop using Capture One Pro...) If you're MAC based and use Capture One Pro you should be okay.
Very nice idea (hardware) and layout. Software sucks if it isn't compatible with your computer, software, or workflow. X-rite does not openly disclose that the Color Munki is NOT compatible with icc version 2.
Btw, Datacolor Spyder3Elite openly discloses compatible platforms and icc verision 2 AND 4.
Samsung 305T (30") monitor. The first three I bought had something wrong with them out of the box. The fourth (I know...what was I thinking) worked for eight months and I am now waiting (3+ months so far) for a return label from Samsung so I can return it for a refund.

I don't have a "worst" purchase as I'm very meticulous about what I get with regards to my needs.
However, I bought Vivitars only to realize it is almost impossible to connect them to CyberSyncs. They don't have PC ports and use a Vivitar specific port. I was trying to mix and match wires, but it was ridiculous trying to find the right cables. Nikon SBs > Vivitars.
Canon XSi/18-55 kit (that I bought for my brother).
Not necessarily a bad camera, but performance was way too sluggish, especially after talking with someone I met shooting with a 40D. Luckily Costco took it back. And for $250 more ($300 minus $50 sales tax) I bought him a 40D/28-135 kit from Amazon. What a huge improvement!! Also, the XSi just doesn't feel right unless you've got smaller hands, which my brother doesn't.
I have two purchases that I can't really say that I regret, but the experience could have been better.
I bought an extremely soft copy of a Tokina 50-150/2.8 -- it ended well, though, because I returned it for the 70-200/2.8 IS.
I'm close to regretting buying three Pocket Wizards. They have been replaced with new ones from the distributor. I haven't had a chance to try them out, but if they misfire they will be ebayed and I won’t look back. I even bought 3 used Nikon flashes because of the supposed Canon radio interference problem... we shall see.
Does a snowblower count Maybe the wraparound aspect of a quartz counter top? New roof on the studio? Wheelchair? An awful lot of hard drives.
Just consulted with the boss who informs me that the only camera thing was the 85L-II - but that was to replace the other one. Though tomorrow I will likely pick up my gift for myself. (maybe post a pic later)
