The idea is to have everybody post information on the adapters they use, so that potential buyers can rapidly acquire the information they need.

Where did you buy it? Eventually leave the name or the link of the ebay shop that sold it.

With which lenses did you use it?
What problems did you encounter?
On what lenses did it work fine, on which others did you have problems?
So here's my take:
For use on Canon EOS 5D:
M42 adapter bought from german ebay shop, Mr. Rolfo, or Phototools (http://stores.ebay.de/phototools24_W0QQsspagenameZMEQ3aFQ3aSTQQtZkm), shop empty at the moment.
Very cheap. Works perfect on Zeiss Jena Flektogon 35mm f2.4, on Asahi Pentax Takumar 50mm aperture is extremely hard with this adapter, infinity focus only at f16, mirror stuck otherwise.
M42 adapter from sinaron.fr, "La Bague Photo", french ebay shop. (http://stores.ebay.fr/LA-BAGUE-PHOTO). The more expensive version (around 30 Euro) works perfectly on the Takumar, but infinity only at f16.
Zeiss/Yashica adapter the same seller, sinaron.fr, "La Bague Photo". 36,90 Euros. Works fine on CZ 28mm, mirror stuck on Planar 50mm f1.7 at infinity, I got problems on the aperture of the 35-70mm after using it, not sure if that was the reason. However, it sits very loosely on the camera, especially with the 35-70. The model has "C/Y" engraved on top of the ring, and dented borders, i think it's quite widespread.
All of this info needs to be on a webpage somewhere. I've been thinking about setting something up but haven't gotten around to asking if there was interest. Not another forum site, but more like a wiki. I think something could really complement these forums rather than compete. The dialogue that goes on here is invaluable.
My only stipulation was going to be that Guy provide a web-rez version of the cockpit by the Zeiss 21
