I would like your opinion for a macro photography setup in the medical field. The most used in dentistry and medical field is the MR-14EX ring flash or the MR-24EX, what happens is that pictures don't look good with the ring flash they seem flat, but the MR-24EX is too expensive i was thinking of setting up a dual flash with bracket.

Generally, "flat" lighting is what is needed for medical/dental photos, which is why the Canon ring flash and devices like the Yashica Dental Eye Camera are/were designed the way they are.
It's not artful lighting, but it gives an easy to see and easy to interpret look at the condition of the tissues being imaged.
Canon MR series is mainly designed for medical/science field. BrianO is right, it's supposed to be like that. If you need something different, try those flash brackets to alter the light angle or maybe some modifiers.
If the macro ring light is too flat, try putting some cinefoil/gaffer tape over half the ring. Could possibly substitute with a little strip of a ND gel? Just guessing here... never tried this myself.

Yes, I would get an ETTL-capable flash. Often, manual power won't go low enough for really close-up photos but ETTL will. You can set an ETTL flash manually, in most cases, if you want manual power for some uses.
