I ended up going through Lensrental.com. I liked their interface and they guarantee the reservation. I had no assurance from rentglass.com that the gear I wanted would be available when I wanted. It was considerably more expensive though, but this event is critical.
I went ahead and got the insurance coverage. Do you guys typically get it?
Ouch.. they don't ship the lenses with filters either. I'm super careful with the gear and will not be carrying it around much, so I'm not too concerned, but you never know when this will bite you. It does raise the cost of rental considerably. B

The reason they aren't automatically added is most people don't want them, take them off, and then forget to return them. This way they're tracked in the inventory control system.
Can't speak for other rental houses, but Lensrentals does NOT charge customers for minor front element scratches. The only time in thousands of rentals we've charged for a front element they were shattered, not scratched. Except for this one time someone used the naked lens to take video of helicopters taking off in the desert and sent the lens back sandblasted.

Yet another vote for Lensrentals.com. I've rented twice in the last few months and they've been great and hassle free. I did have to talk to them to verify my identity for the first rental but that was no big deal and understandable (you could do the verify online but I chose to call instead).
