Well, I am definitely no expert on PS settings. I generally just stumble my way through, so if I am stating the obvious please forgive me. But the first time I have set-up each version everything was on the “main” monitor and I had to drag what I wanted on the second monitor over to that monitor. I think there was someplace to save the workspace setting and then whenever I opened PS again all was as I had arranged it.
Edit>Preferences>Interface>deselect "open documents as/in tabs". Open document, move to second screen. Future documents should now open in the second screen.

Jim, a very belated thank you! It's taken me hours of frustration to get to this solution. I was even using CS5 because I hated my images opening on the wrong monitor. Wonderful!
