Or do I misunderstand how its done, I ask because I am considering a lens and not sure if autofocus is important in this case.

The lens I am considering is Sigma 1800 APO and autofocus might not work on my 5D2 (the lens is old)

In a word: yes. Most macro is manually focused.

Autofocus can be used at magnifications up to life sized (1:1), but it can be a bit of a hassle. It's much easier to manually focus, then sway back and forth until you lock in a good shot. And once you get used to it, you don't even think about it.

Using live view is more critical as far as I am concerned but, that throws you into manual focus if you want a better image.

In choosing a macro lens, AF not important. Consider the focal length and if the front element extends when you focus.

Most macro is MF but not all. I use AF to lock on, then move to/fro if/as appropriate. Given the choice I wouldn't lose it.
