Is that something expected ? If not any idea how to correct it ?

Using the "Edit in .." function from LR, works ok with Elements, that is is presents me with options as seen here
But when choosing CS4 as my external editor, it does not present this option and opens the RAW (.CR2) file in CS4. In LR preferences I have set it this way

First off ... I don't believe you need to, or should "install" ACR in the LR develop module. If you updated LR to v2.4 that is all you need to do ... the ACR plugin is for PS and should be installed there.
In LR it will show the default version of ACR when your camera's file format were first handled by ACR/LR ... even though you have ACR 5.4 installed, it could be an earlier version when your camera was introduced .... that said, you will have all the available setting, options of the latest version of ACR/LR raw development.

I was not expecting to see CS4 opening the raw file from LR, but a TIFF copy , same as Elements.
The simple answer to your question is that Lightroom and Adobe Camera Raw will not work correctly if there is more than one version of the plug-in installed. And apparently you compounded the problem by installing a version of ACR in Lightroom. Lightroom is a standalone program and does not need to be updated with ACR.
My suggestion would be to use Finder/Windows Explorer to search for all occurrences of the Adobe Camera Raw file and delete them. And since you don't seem to know how to manually install the update, open Photoshop and go to Help > Updates and let the updating be performed automatically.
I followed the instructions in the readme file. On the splash screen "About LR .." I see ACR 5.4 . All seems ok. I guess I miss stated something. "After installing ACR 5.4 in LR develop .. " should have been "After installing ACR 5.4, in LR develop ..." which is a different thing.
