I use the appropriate grips on the right of the camera (portrait or landscape), and the left hand under the lens. The weight of the lens remains on the fat part of the hand opposite the thumb - up just short of the focus ring so I can \"touch up\" the focus when necessary with the thumb to the left of the ring and forefinger to the right. This hold also keeps that left elbow tucked in tight for additional stability.
I always leave the tripod ring on, and just rotate it out of the way when necessary. When walking, I find a comfortable location for the ring and use the foot to help cary the weight of the lens on my left hand.
