That's two questions. Here's what I have on the first one.
From the Contax 28mm f/2 review at PeeblePlace,
"The Contax 28mm F2 Distagon’s optical formula is based on a Zeiss 28mm cinematic lens. The cinematic lens design was subsequently adapted for 35mm SLR use, so sometimes the 28mm F2 Distagon is referred to as the “Hollywood 28”. One of the key features of the 28mm F2 Distagon is the floating element design which provides significantly better near field performance and a short minimum focus distance."

It's quite possible that the Cine 27/2 is the same lens, as cine folks seem to be more particular about having exact numerical values, rather than a "where it fits in the lineup" labeling approach. If the Cine 27/2 and Contax 28/2 are the same, then this suggests that the f-stop and t-stop values are both 2, which is certainly possible, but not very common.
The SMC Pentax 28/2 has the same optical formula as the Contax 28/2, and was developed as part of a brief and informal partnership between Zeiss and Asahi Optical (i.e. Pentax).

I think the SMC Pentax should be called the "Vancouver", since that's "Hollywood North".
