It seems that you could put a reflector on the floor, point a light down towards it, place a shoot-thru umbrella over the reflector facing the subject, and drape some black cloth to cut back spill if you need to.

Yeah, there are lots of ways to get the same result.
But I'll bet they sell a fair amount of these things with Scott Kelby talking them up in his blog.
