I figure if they are willing to tell others about my work they must like me enough that I did something right. But no I don't usually send people stuff and I find testimonials that some people have on their webpages to be really annoying because how the heck do I know if they are real or just some made up crap to sell the fotog because the images arn't good enough.

Ask people a lot of questions .. it is the best (and often the only) way to discover how they found you, what they like (dislike) about you, and how they rate your overall product / service.
I do not use a printed questionnaire, but do ask a lot of questions during the whole process .. from initial contact right through to product delivery.
I ask people to rate their vendors (me included) on Wedding Wire. That way they can be honest because they're not directly responding to me, I get more reviews, the other vendors do as well, and the customer gets rewards for doing 5 reviews. Everybody wins.
