yeah man.... I started doing a test because whenever I used a manual lens it's always soft... but bang On when using AF lenses on my 5d2... I found out it was front focusing... while it's really clear and sharp on the viewfinder.

I remember ordering some, or a set of these for my 1Ds series body and a different screen. I don't recall if it was on line and just I tried one of the part #'s in the Canon store and it didn't find it. But he provided a phone number, so give it a shot....

I got the shims from Canon Parts. They had a phone number I used, as I recall. I never ended up using the shims, since I replaced the after market screen with a Canon S screen, and the focus was close enough then. There was a post giving the part numbers for the shims, but you will need to do some searching to find it. I think it was in the Alternative board.
