I am holding a 580 off camera (slaved by a pocket wizard) and then bouncing that off the floor.

I hold the flash in one hand and the camera in the other. After a while you don't notice how bad your hand hurts!
The floor was a very dark concrete color. It does not matter what color the floor is, as long as it reflects enough to not kill my batteries in my flash. I have shot those type of images on wood floors too. We just white balance that light after the fact.
