Fifty dollars on the A7V I bought, and I submitted the form in mid-July. Still just shows being approved, but I have yet to receive it. Has anyone else had this happen?

That seems far too long. I submitted the form for a 35mm GM on Oct 14 and got an approval notice today.

Have you tried replying to the original submission confirmation email?
