I just got this today and am disappointed. My 5d4 with even my 50mm seems to weigh too much. The camera pulls my belt away from my body and just hangs there . It is very uncomfortable. With all these rave reviews I must not be doing something right but I did what the video said to do. I only use the belt option.
Has anyone else tried this option and had problems?
I have the v2 and it works great on a Think Tank Speed Belt with the (discontinued) Think Tank Camera Clip Adapter v3. I'm guessing there are other similar options available. Not surprised a regular belt is inadequate for that size setup.
Cameras I've used it with are 5DSr w/Milvus ZE 21mm, Fuji GSW690II, Pentax 67 (that one is at the limit of comfort).
Peak Design has a "Pro Pad" that helps distribute the weight and keeps the clip and camera from digging into you. It _might_ help a bit with your belt situation or it might not. I put the clip on my shoulder bags, not wear it directly on a belt because it really is pretty uncomfortable.

Carried a smaller Sony mirrorless camera with a compact 35mm lens with the clip attached to my belt and it worked fine. Trekking through rice terraces in the rain, the camera was under my poncho except when taking a photo.
I have a Canon SLR2 with a 24mm STM lens coming so I will try it with that. If that doesn't work well enough back it goes. This is mainly for travel purposes so I would be open to alternatives if anyone has one or two.
I have also ordered the Pro Pad to see how that does.

If that does not work out for you, you may consider a Lumbar Pack that you wear around your waist
