yesterday when I was preparing equipment for the gig and formatted all the cards, one of them gave me error (card not formatted), and I couldn't format it either. The card was working fine just two weeks earlier. I have two 5Dm2 bodies, on the same firmware, so I plugged that card to the other body, and NO error, no issues, reads the images saved on it, formatted the card without an issue. But even after formatting it won't work in the first body.
Is it card error or the camera? No issues with other cards.
Do a low level format in your computer or erase it and reformat using a laxar or sandisk utility.
If it has bad sectors, they will be mapped out. The in camera format does not actually format anything, it just marks the card as empty in the fat tables, and any bad sectors are still there.

If that doesn't fix it, return it or toss it.
I agree, formatting on the computer resolves many card reading/writing errors
