I recently got the KIPON Pentax 645-Fuji G adapter so I could mount a SMC Pentax A 645 120mm f4 Macro Lens on my GFX for scanning film.
The adapter locks into place on my GFX just fine but the lens is not locking in place on the adapter. It rotates about an inch and seems to hit a hard stop. There is a little metal tab on the outside of the adapter that can slide in/out a very small amount but moving that one way or another doesn't seem to do anything.
Does anyone know how the lens should be locking onto this adapter or if there's anything special I need to do? I tried to google this but I'm not finding much information and t he manual for the adapter is quite sparse.

It should just work. I use the Kipon with the Pentax 645 300 f/4 A* without issue. Might try exchanging it. Macro lenses can be weird, though. Have you tried mounting it with the focus ring at different positions, fully retracted versus extended? Or aperture ring at different positions, wide open versus closed down?
I had a Novoflex that was Contax C/Y to GFX and it worked with all my C/Y lenses except the 100 f/2. Sent it back and got a different brand that worked fine. Sometimes they don't design the tolerances to accommodate every variable.

I don't have that adapter, but the adapters should have a small button or tab which acts as a lens release or body cap release as the body caps also lock on P645 mounts.
The 645 lenses also have a mechanical aperture tab that needs to be lined up correctly with a metal prong or similar within the adapter to stop the aperture down. Maybe try putting the lens into A or f11 to mount or at the other end as it might be that the aperture lever is starting on the wrong side of the prong when you are trying to mount the lens.

Thanks for the tips both of you. The adapter does have a very small tab on one side and without a lens that will move in/out a bit on it's own. There's no spring to it though, it just flips whichever way. I can see it moves a little tab on the inside as it moves in/out. I've tried F4 and a mix of other apertures though 32 and even A but it doesn't seem to care what position that is in. I even tried different zoom positions but that doesn't seem to matter either.
I've started an exchange with B&H and hopefully the next one will work better.

I've now looked at online photos of the Kippon and it seems to be missing anything resembling a button on the side to activate the lens release. If it doesn't have such a feature then there is no way to lock the lens on which is very strange.

There is actually a tab on the side that does move a metal tab on the inside of the adapter that looks like it should be what does it - but it just doesn't seem to work for me.

For anyone curious - I got a replacement from B&H and the new one works just fine. Lens locked into place and there's a small tab on the side of the adapter to unlock. Everything works smoothly.
One thing I did notice, and this is the case on the first copy and this new copy. When moving the Pentax 120mm Macro lens into f4, the lowest aperture, it feels a little.. squishy vs the nice clicked feeling each other aperture has. This isn't the case when the lens isn't on the adapter and it seems to just be caused by the tension of the spring in the adapter that keeps the lens stopped down. Not a big deal and it doesn't cause any issues, but just something I noticed in case anyone else ever wonders if that's normal on this adapter.

I have the same experience. It requires a little wiggle to get the blades to move on my 120a. For what it’s worth mine seems pretty solid otherwise. Happy shooting.
