BT is easily subject to any 2.4Ghz interference. Have you tried a 3rd party USB BT transmitter on a short cable to get it away from the computer?

I'd do a delete and reinstall of the software, make sure those Preferences are gone too. Depending on the OS version, you might try a downgrade to a previous version to see if that helps.

I use an Intuos 5 Touch which is several years old uses a Wi-Fi module internally and a dongle plugged into my keyboard and it is smooth as silk:
- macOS 15.6.1
- Wacom Driver 6.3.46-2
