You might consider playing around with the A7R5 camera settings. I don’t know why but I checked mine and I had my AF set to both BBF and shutter - so I had to completely shut off the shutter focus. There’s a plethora of AF options so messing around with different settings might be a fix.

Yeah I've played around with a bunch of different AF combinations, nothing made it any better.

To test this fairly you need the DOF to be similar. That means you want the subject to look the same size in frame. For example on the 50mm, set it at f/2 and have your wife walk from 7m to 1.75m away. Then with the 200/2 walk from 28m to 7m away. This means with the 200mm, the subject should be 4x the distance away from the camera compared to with the 50mm.
I can shoot a head to toes shot of a toddler just 80cm away with a 35GM @ 1.4. This sounds trivial until you realize not only do they repeatedly run towards you, but also at 80cm distance the DOF is only 4cm, and DOF rapidly reduces. At 50cm (half body shot) the DOF is only 1cm.
You are essentially doing the same kind of thing but with a longer lens. In my experience, the A7IV and A7RIV cameras will struggle.

I've had a similar experience with my A7RV and other non Sony lenses, and I always wondered how an A1 or A9 would handle those lenses instead.
FYI, for those wanting to replace the OEM lens foot with an aftermarket one, the Leofoto SGF-02 for the Sigma 500mm F5.6 DG DN OS / 70-200mm F2.8 DG DN OS fits and works great.
The lens mounted on camera no longer will tip back; sits balanced on a table. It's also feels a bit more secure when holding from the lens foot compared to the OEM foot since it has this padded grippy thing on top.

Is anyone using this lens coming from the Sony 70-200/2.8 II? I'm curious whether the tradeoff in flexibility is worth the extra stop and if there are other pros/cons worth considering. I suppose there's the FPS limit and the extra weight as downsides to the Sigma. Both lenses seem universally praised for sharpness. I wonder if a TC on the GM lens actually provides much additional resolution or whether the Sigma can be cropped/uprezzed to a similar degree.

I had the 70-200 II really phenomenal lens but I replaced it with another phenomenal lens, the Tamron 35-150, the focal range just makes more sense for me, but still wanted something with more reach and had the 300GM for a little while but had to sell that. Ended up buying the 200 Sigma and I've been really pleased with it! I don't miss my 70-200 tbh

Did you try setting shutter release priority to AF? This won’t release the shutter until focus is achieved. The other options are balanced and release. Release will just take the picture, doesn’t matter if it is focus.
Any other updates on your combo, curious as I also run a7Rv?

I always set my camera to AF yes, not balanced or release, I'd rather have less pictures but all in focus than a bunch of out of focus pictures. I ended up getting a deal on an A1 II so I no longer have the A7RV and the results are much better. I never did get an answer from Sigma after I tested all the stuff they asked me.

Yeah, the A1 II is such a sweet looking camera. Did you notice any other lenses that had issues? My Sigma 24-70 DNii has a lower hit rate than my GM lenses, but tolerable, and I’m pretty particular about getting focus. My 85DN can also slightly miss focus on moving subjects, I figured the motors were just too slow on that lens.
I'm looking to see how this lens performs in strong backlighting. Does anyone have any sample photos that could help me? 🙏
