Have an a7r, and there's slight bending but not as bad as yours. Pushing up on the lens causes some focus shift, but it's not extreme decentering and only with heavy lenses (I shoot very often with a Sammy 24mm 1.4 so it makes sense for it to sag).
I've heard that you can buy a7s mount parts from Sony? I might do that, and if not I'll consider the Shoten or Tough E, the Shoten seems to have more variability across the frame but fit with more adapters while the tough E has adapter and infinity issues (which most of my adapters are short at infinity anyway so it might be a good trade)

No. The heaviest lens I've used with it is a C/Y Zeiss 135/2.8, and I support that by the lens almost all the time.
How old is your camera?/How old was your camera when you noticed problems?
Purchased in February 2014. It was a floor model, so it might be a touch older than that.
How many pictures have you taken with it?
Somewhere between 15,000 and 18,000. The tool said 10,800, but I know that I've taken more than that.

2nd A7, bought Sept 2015, already 5,500 single exposures.
Rotational play from first and with lenses like C/Y 35-70/3.4; FD 35/2.8 TS, both over 600g with adapter, there is play in every direction, but no gap.
Haven't noticed any affect to image quality, as the "play" always under tension and returns to same point.
Will replace with Tough mount, like the first body.

What kind of play? Noticeable gap when my A7r is mounted with FE 16-35 and easy to wobble with hand pressure.
If you answered the above question with yes: Does/Did it affect image quality? I bought this camera from another FM'er and immediately noticed the wobble, so I replaced the mount within days of getting it with the Tough E-mount LT, which made a noticeable difference (no more gap). I didn't bother checking image quality as I suspected there had to be an impact given the width of the gap I observed.
How old is your camera?/How old was your camera when you noticed problems? I bought it used from the original owner and noticed the wobble immediately. Shutter count when I bought it was around 3,650.
How many pictures have you taken with it? It now has 8,436 shutter actuations according to the tool you provided a link to, so I've taken 4,786 shots and almost all with the new mount.
If you had issues: How did you fix the issue I installed a Tough E-Mount LT.
The gap between lens and camera on the right side explains the tilted plane of focus which I from the start attributed to the lens (Actually three copies of the lens had the same tilt). My conclusion now is that the problem is caused by the sloppy lens mount. I think the A7RII-mount is tighter, and I could do a test to see if my FE55 is straight on this camera.
A quick test of my FE55 on my A7RII shows a perfectly straight plane of focus.
