She's not tall enough to be a model in NYC (about 5'9'' minimum height requirement). But she can do shows, I suppose.
For #1 and #2, it's all ambient from whatever is around. The lighting is from a fountain. EXIF data would show you no flashes. I avoid flash unless the color's horrible or there's no light. It forces me to improve and be a better photographer, in my opinion.

@MVestergaard: It's SOOO much easier to shoot someone who knows that they are attractive and have that confidence.
@bscharf: New York City. It's not a specific place where you can get that kind of lighting. Can find it in a variety of locations. You just need a wide light (think large store display window), a good distance, a good angle on the model, etc.
@Sharona: learning how to use ambient lighting is a challenge in itself!
