For exposure we use evaluative metering on manual for 100% of the day.
For focus we use one shot except for processional and recessional at which point we switch to AI servo.
Almost always manual. I will very rarely use AV, but I do use it for certain situations. I switch 50% evaluative and 50% spot, it depends on the scene.

This is me but I never use any other mode but M while working. I like the total control. Ive never used any of the auto modes except to play and test, dont like it, always needs more post work.
I actually shoot a stop or two over exposed as it works well for my post style.
Manual everything 99% of the time (unless in a drastically changing scene, at which point I will use aperture priority and Auto-ISO). Almost always spot meter and custom WB as well (although lately I've been using my light meter that I use for film for my digital work and incident metering the scene).
Manual mode 97.5% of the time...I like to be in control! Spot metering and AWB. When using flash, always manual.
I would switch to TV if I want to leave the shutter open a little longer to add "motion" and I don't have time to think about M settings. Also, AV if moving from dark to light/light to dark while continuous shooting.
