I just noticed that Op/Tech (via Amazon) offers a screw-in flip-up lens cap. The largest size, however, is only 72mm. And with the thickness of the unit, there could be vignetting on very wide angle lenses. But at least such an item is available to those who want them.

Edit: Optech does make a slightly different version in 77mm.

And I managed to deeply scratch the front element of a 17-55 inside my bag but without the protection of a lense cap.. I was shooting an event and threw my 580ex (metal foot facing down) on top of my face up 17-55. Whoops! My fault, but I did learn how to install a new element, so lesson learned.
I do prefer to keep a lens cap on my lens while it's in my bag. Occasionally I'll throw a hooded lens face down into my bag, but it does get more dust exposure and I rarely use filters. If I had semi-permanent filter mounted, I would be less concerned about using a cap.
I put it in my pocket, but at times was disappointed to find it was sharing space with a tissue. With a lens hood on I find I go a long time without a cap on. Cap goes on when environment gets bad, blowing dirt etc. I will put the cap on when hiking up and down or over steep uneven terrain that I’ve been known to slip (don’t you just love gravity and age!). The cap can add just a bit of protection walking around off trail.

Tethered, but I frequently take it off the lens. When removed, it goes into a pocket. With this system, I've never lost a cap. (even though I keep an extra around)
