Yes, just add the two statements to the existing line that is there in the boot.ini file. What you showed (where you added the statements) is correct. Just do a SaveAs to ensure that you don't change the filename (Notepad likes to add .txt to the end of the filenames otherwise).
### VERY IMPORTANT: The Userva and/ or 3GB switches will not operate correctly if you do not ALSO bump up your virtual ram setting. Some folks say to allocate 2X your physical ram. Others say it's enough to allocate 2X the amount of ram available to the computer (typically around 3.15 - 3.2 gig) so set virtual ram to at least 6.4 gig minimum value.

Also, I've read about Userva thing but did not go that route because I only have 3 Gigs of Ram, so I was not sure what to set the Userva switch at to avoid problems, anyone know?
Dave, I'm not an expert (and besides, the experts all disagree) but I guess you have to figure out how much ram your system needs first, then allocate the rest of your physical ram to your apps with the Userva string. I would think a basic system would need an absolute minimum of 500 meg. Try Userva=2300. That should be safe.
Again, don't forget to bump up your virtual ram minimum-setting to at least 5 gig.
Christ, this is confusing; will CS4 utilize the 3.5 gigs of RAM in 32-bit mode with all this code tweaking or does it need Vista 64-bit? At present my "Preferences" is showing 1.6 gigs installed and I have 4 gigs actually installed. My version of Vista is Home Premium, which is 32 bit based. If I upgrade to 64-bit Vista Ultimate, I know CS4 will utilize the 4 gigs of ram, but can this tweaking 32 bit Vista OSs make it recognize at least up to 3.5 gigs of RAM??

I'd suggest you pay for the upgrade if you want to use all the RAM. In the end you might get windows and CS4 to agree to use your RAM, but its a lot of time and effort spent messing around and as with all workarounds its going to be a bit of a kludge if it does work. In the long run its better to upgrade if you aren't a natural PC tinkerer.
