All Hill & Usher requires is an equipment list. We send them an updated list every few months for our coverage.

Just as an FYI the State Farm policy is not going to provide any liability coverage to you.
If you are getting paid anything, or sell any prints its a good idea to get a small business policy. Very inexpensive, and much broader coverages. Anyone can sue Anyone in this day and age even if there is no merit to the case, the legal costs would be covered by the liability policy, and those costs add up quick.

+1 on going to a local State Farm office... I had a couple of lenses I bought used and simply took them into the office and used their computer to find the comparable lens on the B&H site for proof of replacement value. They simply compared the lens in hand with the lens on the B&H site and that was all they wanted.

But, as others have said, State Farm's policy is not for professionals. It only covers equipment for personal use.

I'm also curious... has anyone has ever made a claim on a State Farm personal article policy?

Tom C. Pickard is surprisingly reasonable and they are very knowledgeable and courteous. They can guide you to the best type of policy for your needs and if you do start shooting for money can help out with things like faxing certificates of liability insurance to the appropriate people. For my policy there all I had to do was list the equipment with serial number and then decide how much "I" wanted to cover it for. There are some items that you may decide the risk is quite low of something happening and may only want partial coverage while other are covered for the full replacement value.

Given State Farm's track record of treating their insureds like crap over the years (forging declination signatures on earthquake policies in Ca after Northridge), they're the last company I'd ever do business with.

Not necessarily true in all cases -- My state farm agent quoted me $375/year for replacement coverage on $5k worth of equipment (all I needed), and it includes $1MM liability protection.
