Yes, one does need to be careful.
I have owned multiple collars made by this company (iShoot) and I have found them to be of good quality. In actually prefer their collars for the RF 70-200 and 100-500 to the factory collars in terms of fit/finish (maybe no great hurdle to overcome).
I also own this particular collar and can clarify that it fits well and does what it says. However, as this particular lens has no "groove" for a collar, it is attached to the lens solely by the friction of the clamp. As Canon RF lens barrels are made of engineering plastic, I strongly recommend not over-tightening the collar. I would also never carry the camera+lens with a strap attached to the collar's foot.
The inside of the collar has a nylon ring intended to minimize any marring of the lens barrel's surface; however, some signs of use will likely become visible. Canon RF lenses tend to show surface marks easily, in my experience.
Parting thought for the OP: only consider this if you truly intend to shoot on a tripod most of the time. For hand-held use, this collar makes an already awkward lens even moreso. If you will be transitioning to and from a tripod in the field, you may find it inconvenient.
