The narrow depth of field is an illusion. The background is a bit frenetic and soft from using a low shutter speed, purposely moving the camera but then using flash to make the foreground subject static. The flash didn't reach far back enough to 'freeze' some of the elements in the background thus the illusion. It's an old technique that I use on occasion.
