When I buy a Nikon gear, I have to make sure it is "US" not "import".
Do I still have to look at this weird thing when I buy a Sigma lens (for Nikon), US vs import?

I think US vs Import for Sigma makes a difference in terms of US warranty. But, unlike Nikon, I haven't heard about Sigma refusing to repair Import models even with payment. So if you don't care about warranty coverage, buying Import may not be as risky with Sigma.

Sigma's site does suggest that they will repair gray market items, obviously with an appropriate charge:

"Gray Market
As of January 1, 2017, any product that is not imported by the Sigma Corporation of America or purchased from an unauthorized Sigma USA Dealer will not be serviced under warranty regardless of the service required. The Sigma Corporation of America Service department will service these products for a minimum $250 charge in addition to the required parts and labor charges at the owner's expense."
