There's a first time for everything - actually this was the second (2 of 2) frames showing this.

All others were fine.

SOOC - well, RAW to JPEG on export in LR

16:9 frame overlaying two other frames?

I've been this with a bad card before but I've also seen it on a 5DM3 that ended up requiring an I/O PCB replacement. To help distinguish, were you shooting in single-shot or multiple? Using both CF/SD slots at the same time?

Shooting full bore - writing only to the SD as the CF had just filled (Auto Switch Card). This was visible on the LCD upon review so I paused, changed CF cards and went back to writing to CF.

The problem on the 5DM3 requiring the PCB replacement also occurred when shooting continuous - wouldn't happen for single exposures. I would switch to a different SD card and keep an eye on it; if you see the problem again I would send the body in.

Have you fully tested the SD card in a computer?

No. As in a read-write test?

If you have access to a Windows machine, I have found the program "USB Flash Drive Tester" to be helpful in the past: http://www.vconsole.com/client/?page=page&id=13

Have you re-transferred the images to your computer? Sometimes my card reader has a digital hiccup and corrupts a couple frames in a similar manner to this. Retransferring them fixes the issue.

No, I haven't. The card has been reformatted and the glitch occurred during the write in camera - I could see this image on the LCD as is. I have downloaded the utility Damon mentioned and will try it out later today when I get home from work.
