Not used them (as I'm a uk resident) but MIDSTATE seem to crop up alot when it comes to old canon repairs . They seem to do alot of pipes and 200/1.8's
The AF/MF selector is a common issue on the pipe . Its something I try to touch very little on my pipe . When I'm not using the lens I try to leave it in 1.8m AF. That way if the thing does fall out I still have full use of the lens (I don't MF with it anyway )
Good luck finding a fix . A lost selector is a horrible reason for a great lens like the pipe to die
