Oh, reaaly? I use 'em. They're for when you're doing straight-on flash and want to avoid redeye.

Nah...stofen, or bounce card or something...no brackets since invention of tupperware ... (AKA Garry Fong Dome)

Actually adds up extra wight.. To me this is old school..but it is personal though. I just shot in April with a guy who still use them.

Brackets also keep the flash pointing at the same place when switching between landscape and portrait orientation, which is rather handy when bouncing off walls or ceilings. The cable, however, is annoying.

My minimum is what I bring everytime. I used to lug everything but after not using any of it I sold off most superflous lenses and gear.

1 5DMKIII w/ 64GB CF and 64 GB SD set to duplicate
1 7D for backup / reach
Tokina 16-28
35L
70-200 vII
40mm pancake kept on backup as a "body cap"
baggy full of camera batteries and spare eneloops
4 x 600exRTs
2 x light stand / massive brolly
2 x manfrotto clamp flash thingies to clamp flashes to railings

I could do most weddings in a pinch with only 5d, 70-200 and 35L though if need be.

The bare minimum I would be comfortable bringing to a wedding:

2 bodies (one being a backup)
35 or 24
85 or 70-200
2 speedlights

That said, what I usually take is:
5d3 (x2)
24L
35L
85 1.4
100 macro
135L
70-200 2.8IS mkII
600ex (x3, soon to be x4)
LED panel
last wedding we were restricted for the ceremony so we brought the 300 2.8L

1DsIII - 24/70 - 580exII - CP-E3 - 64gig/16gig - 2000shots

1DIII - 70/200L IS - 430ex - 32gig/8gig -2000 shots

Spares - 35L - 24/105L IS - 77 ND - 77 CPL - 2x270ex - ST-e3 - 2xLP-E3 - 32 Eneloops - 32/8 + 16/4 cards.

Everything fits in a hard shell lockable bag and stays with the band or DJ.

Two bodies and generally two lenses with four flashes and PWs and stands.

Changing lenses is not what I want to do at a wedding. What I do want is OCF to light a scene that creates dramatically different images than all the DWACs at the usual wedding these days.
My 70-200 offers plenty of shallow DOF as does the 24-105. High ISO provides the flexibility in the darker situations.

I will have some spares in a case stored at the site but I do not want to spend 10 hours on my feet with ton of gear.

BTW, one can use LR to see precisely the lens usage you have on a particular wedding or in your entire library. I use this tool to talk me out of a "sexy" FL that in the real world I never use. Sadly it is also talking me into a 135 L for portraiture.
