A cousin of a good friend of mine in Bangalore, India did this. I am guessing he used TS-E lenses, but not exactly sure.

Any ideas?

@vsg28,
Very slick... this is likely done with a tilt-shift lens with a reverse (upward) tilt. Doing so isolates a very narrow focal plane and causes the rest of the image to blur into bokeh nothingness.

TS-E lens for sure but, possibly, some selective blur added in post as well using After Effects or some other program. Very well done either way, thanks for sharing that...
