What is the purpose of setting the distance indicator display to feet? Nowhere on the flash display can I find anything displayed in feet. I know the Nikon SB-900/910 displays the distance in meters and might be able to display in feet, but can't figure out if and how to get the 580 EX II to display distance at all. I'm pretty sure it can be displayed in ETTL mode, but can't figure out how and would rather shoot in manual anyways. What am I missing?

It should always show when it's in normal mode, whether ETTL or Manual. If it isn't, then you're still in a function-setting mode or something.

You'll also need to have the flash on the camera and half-press the shutter button once to transmit the aperture and ISO info to the flash.

Do you have a User Guide?

My user guide is in storage...I'm in Korea now. I have Syl Arena's Speedliter's Handbook though, which I've read cover to cover, but can't find it in there of what I'm doing wrong.

I have the 580EX II mounted on my 5DM2 with the following settings:
Camera:
Manual exposure
ISO 100
Tv 1/125
Av f/5.6
RAW
Evaluative metering
One Shot AF
Single Shot
White Balance Flash
Picture Style Portrait

Lens:
50mm f/1.4
AF On

Flash:
Custom Function 00-1 to convert to feet (which is not displaying)
Mode Manual
Zoom 50mm
Zoom Manual
Master Ch 1
1/128 power

Flash is mounted in camera hot shoe

On a side note, my 3 ft Vello TTL cord would not fire the flash when mounted on the 5DM2, but does (last I checked) on my 30D. I have a 24 ft Flash Zebra TTL cord that works on both camera bodies.

I also can't get it to switch into second-curtain sync. It will only cycle from normal mode to high speed sync....no second-curtain sync.

I think you may be in Wireless mode.

I was reading the manual and went right past the part that said second curtain sync doesn't work in wireless mode, but didn't catch it. Now that works as well.

Wireless mode allows the flash to act as either a Master or Slave, and control or be controlled by another Speedlite.

When in wireless mode, there's a whole slew of commands that have to be sent back and forth between the various Speedlites, and so if you're not using a Slave, the Master gets confused when it doesn't receive the talk-back from the non-existant Slave. I think that's what was happening to you.

That's what I thought....I tried turning it off, but couldn't figure out how while using the flash. I disabled it via the camera menu. I love this body...my 30D can't control off camera flash.

I'll have to give it a try tomorrow with my 430EX II as a slave, but do you know if the distance meter will work then?

I don't think it will. The User Guide goes into detail in the section on Wireless.

Also, I went from a 20D to a 7D and found as you have that controlling the flash from the camera menus is great. There can be conflicts between what's set on the flash and what's set on the camera, but if I recall correctly the camera menus over-ride the flash when the flash is on the camera.

On the original 580EX, changing from normal to Master or Slave was done with a simple lever on the bottom of the flash. With the EX II you have to use menu commands, which on the flash means holding down the [Zoom] button for several seconds to activate the settings mode and then using assorted turns of the wheel and presses of the buttons.

Using the camera menus -- on cameras than can do it -- is much easier.
