It's hard to recommend Kelby when he puts Photoshop Disasters on the covers of his books and tries to pass himself off as a Photoshop guru:

Martin Evening is much better for Photoshop. He has a book out for every edition of Photoshop and they are specifically designed for photographers' interests and needs.
For general photography there is Langford's textbook, now in 7th edition. The basic one covers a lot of stuff, and there is also an Advanced one.
For lighting, there is "Light, Science, and Magic".
For portraiture, see Karsh, Don Peterson, some Amherst Media books, Monte Zucker, Arnold Newman and many others.
