I have no real statistics but most used 4 were for sure:
24 1.4 II
100L macro
70-300L IS
300 2.8 IS
It's hard to say, I did start taking a ton of macros with the 100L but then again I did fire off a lot of shots at one soccer game and a week's worth of surfing with the 300 (and other stuff but not enough for the other stuff to matter in terms of most used) but my main two walk-arounds were the 24 and 70-300 so I'm not really sure, i have a feeling it wasn't the 24 though even though it was used a decent amount and yeah I think I can eliminate that one.
For now I'll have to just say 100L, 70-300L or 300 2.8 IS. If I had to guess I might go with the 70-300L. I did take that to a few in the stands games and used it the most on the VT trip so that probably piled on shots for it too.
So 70-300L IS?
(I think 70-300L for sure on the 5D2 and 300 2.8 IS on the 7D although the 100L also saw a lot of 7D usage but as much macro as I shot it's not like I snapped away like mad other than a few times for natural light dragonflys as for sports so yeah I think 300 2.8 IS must have been most used on 7D for the year)
(of lenses that I have ever personally owned my single least ever used lens was the 24-105L)
24-105 f/4L is used the most.
70-200 f/2.8L IS II is my favorite, and used the second most (only because it's less versatile, and heavier than the 24-105).
500/4 is spectacular for a narrow purpose, and used a lot for that

70-200 2.8L without even looking at LR, but I just got a brand spankin new 24-70 2.8 L and I think this will be very well used by end of 2012. I can't believe how I got along without it.
Looks like 400 DO is it, although not by a wide margin.
Actually, shouldn't be surprised by that......400 DO is a very handy lens, relatively easy to carry around and shoot with.

Hard to tell precisely, because it's not clear where you draw the line. Something in the range of 500k-800k frames. Note this does not include the first lapse because that was in 2010.
For comparison, I have kept 750 family photos for the year. So it is about 1000:1.
I knew the winner and runner-up, but I had to look in my LR stats for 2011 anyway to see how the rest of the lenses were doing. Bodies used were 5DmkII (dominantly), 1DmkIV, and (for a handful of shots) 40D:
1) 35L: 35%
2) 100L: 22%
3) 24L mkII: 15%
4) Sigma 150mm with and without 1.4x: 6,4%
5) 85L: 6,1%
6) MP-E 65mm: 5,5%
7) 300mm f4 IS + 1.4x: 3,9%
8) 135L: 3,3%
9) 14L: 2,0%
The 300 bare, the Sigma 15mm fisheye, and excellent TS-Es got only marginal use.
